I have been informed of, understand, and am aware that strength, flexibility, and aerobic exercise including the use of equipment, is a potentially hazardous activity. I also have been informed of, understand, and am aware that fitness activities involve a risk of injury, including death or serious disability, and that I am voluntarily participating in these activities. I hereby acknowledge that I am voluntarily assuming the risk of any such injury, disability or death on behalf of myself and my child. I hereby waive, and release and hold harmless on behalf of myself and my child and our respective estates, heirs, successors and assigns , Mommy and Me Fitness Inc., it's officers, directors, employees, agents and volunteers and the Church/Organization who sponsors this program from any claims, demands, injury, damage, lawsuits, expenses (including but not limited to reasonable attorneys fees) of any type, whether known or unknown, arising directly or indirectly out of my participation and my child's participation in the Mommy and Me fitness program. I specifically acknowledge that I have read carefully and fully understand this agreement and that I make it freely. I also certify that my child and I are in good physical condition, and are freely able to participate in an exercise program.
